📄 A. Portal Printouts (VERY IMPORTANT)

You must print:

1. ✅ Invitation Slip (with assigned table)

2. ✅ Application Submission Slip

3. ✅ Guarantor’s Form (completed and signed)

4. ✅ Credential Screening Form (if shown on your portal)

5. ✅ Physical Screening Form (if shown on your portal)

⚠️ The Invitation Slip is the most critical.

📄 B. Personal Credentials (Original + Photocopies)

Carry originals and clear photocopies of:

1. ✅ NIN printout or NIMC card

2. ✅ O’ Level Result (WAEC / NECO / GCE / SSCE)

3. ✅ Birth Certificate or Declaration of Age

4. ✅ Certificate of State/LGA of Origin

📄 C. For Specialist Applicants ONLY

Bring:

✅ Trade Test / Professional Certificate

OR certificate from your training (your oga certificate)

👉 If you applied as General Duty, this is not required.

📄 D. Guarantor Requirements

Your guarantor form must:

✅ Be fully filled in handwriting

✅ Be signed by qualified guarantors

✅ Have guarantors’ passport photographs attached

✅ Have photocopies of guarantors’ official IDs

⚠️ Incomplete guarantor form can disqualify you.

📂 HOW TO PACKAGE YOUR FILES (VERY IMPORTANT)

You must prepare:

✅ Two white flat files

Inside them:

One file → Originals

One file → Photocopies

Also:

✅ Attach at least four recent passport photographs

✅ Arrange documents neatly in order

✅ Write your Reference Number clearly where required

👕 DRESS CODE FOR SCREENING DAY

You MUST wear:

✅ White T-shirt

✅ White shorts

✅ White canvas shoes

✅ White stockings

⚠️ Do not ignore the dress code.

As of the latest PSC notice, these are NOT compulsory unless later announced:

❌ Medical fitness certificate

❌ Attestation letter

👉 Only bring them if your invitation slip specifically asks.
