Post-Pearl Harbor investigations identified a systemic failure of intelligence coordination — multiple agencies held individual warning indicators that collectively pointed toward imminent Japanese military action, but no central body existed to synthesize them. The CIA, established in 1947, was the direct institutional response. The 9/11 Commission Report in 2004 found a virtually identical failure — intelligence agencies possessed warning indicators not effectively shared or synthesized, and coordination failures again contributed to a strategic surprise. The Intelligence Reform and Terrorism Prevention Act of 2004 and the Office of the Director of National Intelligence were the institutional responses, parallel in purpose to the post-Pearl Harbor reforms. The recurring failure of intelligence sharing despite institutional reforms designed specifically to prevent it remains one of the most studied problems in national security organization — a problem America has now addressed twice after paying for the lesson in lives both times.
