UPDATE: We're officially in the "soft opening" phase and are booking Physical Therapy patients!!! We are temporarily "cash pay" due to waiting for insurance companies to certify us as ”in network.” We’re at their mercy with the timeline for that. The office phone number is (812) 420-3355 . Doctors can send orders through EMR, script, or fax 949-703-8006 as well. Please be patient with us as we work out the kinks of opening :).
