Job description
Looking for dynamic experienced or in-experienced-but-certified (entry-level) Medical Assistants who are interested in joining a cool & innovative digital health company that is revolutionizing the way healthcare is delivered.
Location: Hybrid (mostly remote) | Timing: Flexible | Work
Commitment: Full Time (preferred) / Part Time / As per your convenience
Earnings: Based upon number of patients (min. 100 patient signup required).
Job Description:
1> Work closely with providers to understand the health condition and remote health monitoring needs of the enrolled patients.
2> Set up the vital target values and notification / alert rules for each patient in consultation with the providers.
3> Monitor vital data for each enrolled patient and look for early intervention indicators.
4> Monitor alerts / notifications for early intervention.
Conduct maximum of two monthly check-in calls for each compliant patient.
5> Motivate the patients to be compliant by educating them about the importance of monitoring vitals.
6> Coordinate and Communicate with the provider for any required intervention and coordinate any telemedicine call between the patient and the provider.
7> Communicate with stakeholders (providers/care team members / family members / company members) in case of any critical alert or early intervention indication.
8> Document all the monitoring and check-in activities.
Follow up with non-compliant patients.
9> Co-ordinate with provider / other care team members for any issue.
10> Check Call to update care plan and monitor vitals
11> Track & improve health KPIs / Metrics to improve health outcomes.
Strictly follow HIPAA compliant security & privacy policies and regulations.
Qualification & Experience: Medical Assistant Certification or Experienced Medical Assistants
