When people hear the term case management, it can sound abstract or administrative. But in homelessness services, it's one of the most human parts of the work.
Case management is ongoing support rooted in trust. It's the relationship that helps someone navigate housing, healthcare, income, employment, and personal goals while working toward stability.
There is no single path forward. No checklist that works for everyone.
Instead, case managers meet people where they are, helping them navigate challenges, access resources, and take the next steps toward the future they want for themselves.
Because lasting change doesn't happen overnight. It happens through consistent support, meaningful relationships, and a belief in each person's potential.

We are so grateful to the staff, partners, and community members whose care and commitment made this work possible.
After opening new sites, the City of San José has decided to conclude the Navigation Hub program, effective December 31, 2026. Over the coming months, HomeFirst will participate in a thoughtful wind-down process focused on minimizing disruption and supporting continuity of care as participants transition into other programs. The City will offer all current participants placements in alternative program options.
Over the past year, the Navigation Hub has supported more than 250 individuals experiencing homelessness with shelter, care, and pathways toward stability.
As a pilot program, it offered low-barrier shelter, daily meals, space for companion animals, and onsite support, including case management and housing navigation. Staff walked alongside participants as they worked toward their goals—support that has already helped so many individuals move into stable housing.

This weekend marked the Summer Solstice, and as temperatures rise, so does the need for life-saving support.
Warm weather kits provide essential protection from extreme heat for people experiencing homelessness—helping prevent dehydration, heat exhaustion, and other heat-related illnesses.
We are inviting corporate partners to join us in hosting warm weather kit builds this summer. These hands-on volunteer events are a powerful way for teams to create direct impact while supporting community members facing extreme heat.

Casey is a veteran, a member of the LGBTQ+ community, a person in recovery, and someone with lived experience of homelessness. His journey reflects both the challenges many people face and the resilience it takes to keep moving forward.
