Note: This doesn't mean that if you're noticing one of these signs your client is leaving you. But if you notice one or more signs consistently over a long period of time(depending on the contract between you and your client) you should clarify things with your client and seek to improve on your services if the fault is from you.

Also, you're not always the problem. E.g the points mentioned in numbers 5&6, are clear examples where it's not your fault. So in all your dealings, render quality services to your clients and try your best to profer solutions to their specific problems.
