The UK water sector has made real progress on safety over the past decade. Better monitoring, stronger reporting processes, and safety cultures that look truly different to where they were twenty years ago.

Every serious incident investigation ends with the same implicit promise. We looked at what happened, we understood why, and now we have taken action to prevent it from happening again.

But that promise depends on human understanding, not a report.

As AI takes on more of the analytical work in investigation, it's worth asking whether that understanding is still being built in the people who need it most.
