It all depends on patients history and how deep you look. Always look for what kind of injuries they had in past. It can be 20 years old car accident or surgery that is now causing so called new pain. But its not new pain, it's just been hiding through all this time. We all have compensations, some create pain some don't and when they do we should aim to find the root cause.
