1) VALID original and photocooy of Philippine Passport
2) Certified copy of Approval of Retention or Reacquisition of Philippine Citizenship (Para sa mga Dual Citizen)

If nagpalit ng pangalan or correction of entries and/or discrepancy:

1) VALID Philippine Passport
2) Original PSA authenticated documents na nagpapatunay ng change of name
- Marriage Contract
- Annotated Birth Certificate
- Death cerificate of spouse
- Annotated Marriage Contract to show divorce/annulment/court ordered instruction
