• PSA Birth Certificate (4 photocopies)
• PSA Marriage Certificate (for married female applicants – 4 photocopies)
• TESDA picture (with collar, matte finish) (Get it from Photoline)
• 4 pcs. 1x1 ID
• 6 pcs. passport size
• Any High School Record (Diploma, TOR, or Form 137) – 4 photocopies
• 1 Long Brown Envelope and 1 Plastic Envelope
