Ang PhilHealth On-Site Registration ay isasagawa sa ika-8 ng Hulyo, 2023 araw ng Sabado simula 9:00 ng umaga hanggang 2:00 ng hapon sa Brgy. Hall ng Santiago.

REQUIREMENTS FOR PHILHEALTH REGISTRATION/UPDATING:
-Properly Accomplished PhilHealth Member Registration Form (PMRF) in CAPITAL LETTERS
(Mahalaga pong punan lahat ng hinihingi ng nasabing form gaya ng buong pangalan, pirma, contact number atbp.)

Para sa Self-Earning Individual/Voluntary:
-Photocopy of Birth Certificate with Registry Number or any valid Government ID
-lsulat po ang PROFESSION (pinagkakakitaan) at MONTHLY INCOME

Para sa Senior Citizen:
- Photocopy of Senior Citizen ID o Birth Certificate

Para sa Pagdedeklara ng Legal Dependents:
a.) Asawa (na hindi active PhilHealth member)
-Photocopy of Marriage Certificate with Registry Number
b.) Anak na edad (20) taong gulang pababa
-Photocopy of Birth Certificate with Registry Number

Para sa pag Change Status:
- Photocopy of Marriage Certificate with Registry Number

Para sa Correction of Name/Birthday/Sex:
- Photocopy of Birth Certificate with Registry Number or (2) valid IDs

Samantala, patuloy naman po ang PhilHealth DROPBOX REGISTRATION mula Lunes hanggang Biyernes 8:00am to 5:00pm sa:

• Magsasagawa din po ang PHILSYS-PSA ng:
-NATIONAL ID REGISTRATION - para sa lahat ng 5 taong gulang PATAAS.
-EPHILD ID - kung wala pa kayong national id maari kayong mag request nito sa aming tanggapan dalhin lamang ang transaction slips
-NABUBURA NA NATIONAL ID - dalhin ang national id na katunayang ito ay nabubura.
-WALA PANG BIRTHCERTIFICATE - dalhin ang mga sumusunod:
-Baptismal cert
- VRR from Comelec
-form 137
-affidavit of 2 disinterested person -affidavit of delayed registration of birth
-valid ID's.

[BARAS PRIDE: Team Building Activity / Volleyball One Day League] Matapos ang isinagawang Learning Session ay nagtagisan naman ng lakas at galing sa volleyball ang miyembro ng Baras PRIDE kaisa ang mga allies (tagasuporta) nito noong ika-15 ng Hunyo, 2023 sa Baras Gymnasium.

Ang Volleyball One Day League ay bahagi ng capacity enhancement sa Baras PRIDE na siyang nagbigay daan sa pagbuo at pagtuklas ng mga bagong kasanayan at kakayahan ng mga miyembro nito. Kaugnay nito, hangad naman ng gawain na bigyang pagpapahalaga ang diwa ng sportsmanship at mabuting pakikipag kapwa.

LGBT Dancesport Competition

Performance Date: June 30, 2023
Call Time: 3:00pm (the participants are required to join the Pride March)
Venue: Baras Municipal Gymnasium

II: GUIDELINES
1. The competition is open to Baras LGBTQIA+.
Preferably: a pair of both gender (eg. gay to gay / le***an to le***an)

2. Only the first ten (10) pairs will be accommodated.

3. There will be three (3) Latin American dance styles which includes Cha-cha, Rumba, and Jive which will be performed by each pair for a total of six (6) minutes.

4. For the Finale, there will be a Derby Showdown of all pairs using a random latin music.

5. The pair shall wear appropriate dancesport costumes and footwear.

6. Dangerous stunts are STRONGLY DISCOURAGED during the competition. A funny stunt should be included but will not conclude each dance styles.

7. The official competitors shall attend the program orientation and draw lots on June 15, 2023, 9:00am onwards at Baras Gymnasium. Pair attendees will get additional one (1) point on the final score.

8. The decision of the judges is FINAL and IRREVOCABLE.

III: CRITERIA FOR JUDGING:
20% - Posture and Balance -includes coordination, bodyline, shape
20% - Quality of Movement - includes flow, weight, time, space
20% - Movement to Music - includes timing, musicality, rise and fall, sway
20% - Partnering Skills - includes physical/ visual communication
20% - Choreography and Presentation - includes creativity, interpretation and entertainment

IV: PRIZES AT STAKE:
1st Prize - 12,000.00 and Certificates
2nd Prize - 9,000.00 and Certificates
3rd Prize - 7,000.00 and Certificates
7 Consolation prizes - 5,000.00 and certificates
