📢 ANNOUNCEMENT | TES 3B REIMBURSEMENT (Batch 2023–2025)

Attention: ADSCO TES Grantees from Batches 2023, 2024, and 2025!

The ADSCO TES Office is now officially accepting applications for TES-3B Reimbursement. Please review the eligibility criteria and requirements carefully.

⚠️ ELIGIBILITY
This reimbursement is exclusively for TES Grantees who graduated in 2023, 2024, and 2025.
Applicants must have applied for or taken their respective Board Examinations.

📝 REQUIREMENTS & PROCESS

1. Google Form Registration
All applicants are required to complete the official tracking form:

2. Annex 3 – Letter of Availment for TES-3B

Download, accomplish, and print two (2) copies
Use A4 size paper only

3. Official Receipts (ORs)

Submit the original and one (1) photocopy of all official receipts covering your board exam-related expenses

Note: Breakdown of expenses must be reflected in Annex 3

📌 ADDITIONAL REQUIREMENT FOR TES 3B APPLICANTS
All applicants are required to submit the PRC Licensure Examination Application Form (from LERIS).

📄 How to get your Application Form:
Log in to your PRC LERIS account
Click “Existing Transaction”
Find your exam application transaction
Click “Select Transaction”
Click “Print Application Form”
Download and print the form

⚠️ Important:
• This serves as your Licensure Examination Form
• Ensure the printout is clear and complete
• Submit together with your other TES 3B requirementsannouncement
📁 SUBMISSION GUIDELINES
Place all required documents in a short brown envelope
Ensure documents are complete, accurate, and properly arranged to avoid delays
