b. Senior High School Certificate
c. Certificate of Good Moral Character
d. Birth Certificate (PSA authenticated) – 2 copies
e. Marriage Certificate (PSA authenticated) – 2 copies (for married females)
f. Short Brown Envelope – 1 piece

II. Transferee
a. Certificate of Honorable Dismissal
b. Transcript of Records for Evaluation
c. Certificate of Good Moral Character
d. Birth Certificate (PSA authenticated) – 2 copies
e. Marriage Certificate (PSA authenticated) – 2 copies (for married females)
f. Short Brown Envelope – 1 piece
