LET'S GET VACCINATED 💉

For 𝗔𝗱𝗱𝗶𝗻𝗴 𝗮 𝗩𝗮𝗰𝗰𝗶𝗻𝗮𝘁𝗶𝗼𝗻 𝗥𝗲𝗰𝗼𝗿𝗱 on 𝗛𝗜𝗠𝗦 follow the steps provided below.

Step 1: Go to url and type https://hims.ustp.edu.ph/. Click 𝗟𝗢𝗚𝗜𝗡 then Select 𝗣𝗔𝗧𝗜𝗘𝗡𝗧.
Step 2: Log on to your 𝗛𝗜𝗠𝗦 𝗔𝗰𝗰𝗼𝘂𝗻𝘁 From your dashboard, click 𝗩𝗔𝗖𝗖𝗜𝗡𝗔𝗧𝗜𝗢𝗡 then click 𝗔𝗗𝗗.
Step 3: A pop up will then appear, 𝗙𝗜𝗟𝗟 𝗢𝗨𝗧 𝗬𝗢𝗨𝗥 𝗩𝗔𝗖𝗖𝗜𝗡𝗔𝗧𝗜𝗢𝗡 𝗥𝗘𝗖𝗢𝗥𝗗 𝗜𝗡𝗙𝗢'𝗦 then click 𝗦𝗔𝗩𝗘.
Note: You may fill out the 𝗗𝗮𝘁𝗲 𝗼𝗳 𝘆𝗼𝘂𝗿 𝗡𝗘𝗫𝗧 𝗗𝗢𝗦𝗘 when you have received it.
