* Allotment Slip
* Workers Information Sheet
* Embarkation/Disembarkation
* Proof of Billing
* Proof of residence or Barangay Clearance
* CB Docs/Valid ID
* Screen shot of FB account
* Marriage Contract (if married)
* Send an AUTHORIZATION LETTER to the
CREWING MANAGER for verification purposes

Additional requirements for Pinoy Sea Based Loan

* Latest Payslip (atleast 3 months)
* Previous remittances from CB (atleast 3 months)
