📢 Important Update: ID Releasing Schedule

Please be guided by the following schedule for claiming your IDs to ensure a smooth and organized process:

Student IDs: Every Monday – Friday

Alumni IDs: Every Saturday ONLY

Time: 8:00 AM – 5:00 PM
Where: Management Information System (MIS) Office

ANNOUNCEMENT!

ID RELEASING WILL RESUME on Monday, March 9!

Students may proceed to the Management Information System (MIS) Office starting that date to claim their official ID cards.

⚠️ FOR STUDENTS WITH SPECIFIC CASES:

If your ID is currently damaged, or if you have lost it, please follow these updated protocols for replacement:

1️⃣ For DAMAGED IDs:
A PHP 100.00 reprinting fee must first be settled with the Cashier. Please bring your damaged card and the official receipt to the MIS Office.

2️⃣ For LOST IDs:
A notarized Affidavit of Loss is required and must be presented at the MIS Office (Note. Affidavit of Loss are available at the School's Law Office for P100). You will also need to proceed to the Cashier to pay the reprinting fee.
