a. Personal Data Sheet (2017 Revised Form) MUST be properly accomplished and notarized;
b. CS Form 212 (Personal Data Sheet AND Work Experience Sheet);
c. Copy of Transcript of Records & Diploma;
d. Certification of work experience/s;
e. Copy of appropriate eligibility; and
f. Copy of related training/seminar certificates
