The Guidance and Counseling Office of ISU Angadanan Campus is now implementing an Integrated Guidance and Counseling Management System to ensure the safe and organized storage of Student Cumulative Records.

1. Original copy of Grade 12 Report Card (Form 138)
2. Certificate of Good Moral Character
3. Photocopy of Senior High School Diploma
4. Photocopy of PSA/NSO Birth Certificate
5. Four (4) copies of 2x2 ID Picture (white background and name tag)
