Just bring the ff:
1.Vaccination card
2. Valid ID
3. Pen
*if Pedia 12 to 17yo with parent
4. Birth certificate
5. if with guardian only: Authorization and guardianship

Interval of booster:
➡️ 1st Booster - 3 months interval from the 2nd dose
➡️ 2nd Booster - 4 months interval from the first booster
➡️ Pedia booster - 5 months interval from the 2nd dose.

MSCC Vaccination Schedule
➡️ Where: Malolos Sports Convention Center
➡️Time: 8am to 3pm or until supplies last
➡️When: July 4 -8 , 2022

All Senior Citizen, medical frontliner and Immunocompromised (cancer, HIV patients, pts taking steroid medicine, autoimmune disease , dialysis pts) can now receive their 2nd booster shot.

Recommended interval of the booster shot

✳️ 1st booster shot - 3 months onwards from the primary shot.
Vaccine available: Moderna
✳️ 2nd booster shot - 4 months interval from the 1st booster shot.
Vaccine available : Pfizer and Moderna.

✴️ IMMUNOCOMPROMISED- 3 months interval from the 1st booster shot .

Vaccine available for 2nd booster shot: Pfizer and Moderna

Requirements:
☑️ Birth certificate, baptismal or form 138
☑️ 1 valid ID of parent or guardian
☑️ Authorization letter and certificate of barangay for legal guardian (if parents is not available)
☑️ Medical Certificate for person with comorbidity (e.g asthma, g6pd, heart problem etc.)

🖤 1 VALID ID OF PARENTS/ GUARDIAN
🖤 Authorization letter & Certficate of Barangay for legal guardian ( no parent/s available)

If having comorbidity (e.g. asthma, g6pd, heart prob etc.) We also required medical certificate in their doctor.
