ADMISSION REQUIREMENTS:
* Form 138 (Grade 12 Report Card) or its equivalent.
* Certificate of Good Moral Character issued by authorized school official.
* Five (5) Recent 1×1 ID colored pictures
* SSC-R College Admission Test (SSC-RCAT) Result, except for high school graduates of Recollect Schools.
* Testing Fee (P400.00).
* Birth Certificate in an NSO Security Paper (photocopy)
