1. Basic Leadership Skills Training Certificate
2. High School Report Card (for High School Graduate) or TOR (for College Undergrad) or ALS Certification and Rating (for ALS Passer)
3. Authenticated Birth Certificate
4. Barangay Certificate of Low Income (Original & Photocopy)
5. Certificate of Good Moral Character (Original)
6. Medical Certificate (Original)
7. 1 pc. 2x2 ID picture (formal)
8. 1 Recent Family Picture
9. 1 House picture

The Joint Delivery Voucher Program is a Department of Education initiative designed to assist Senior High School students enrolled in the Technical Vocational Livelihood (TVL) track in gaining appropriate industrial or workplace exposure.
