1 Cover Letter
2 Resumé
3 Personal Data Sheet (w/ attached Work Experience Sheet)
Photocopies of the following:
4 College Transcript of Records
5 College Diploma
6 Proof of Eligibility:
- PRC Licencse
- Certificates of Board Rating, Good Standing and Passer
7 Certificates of Trainings and Seminars Attended
8 Any Proof of Teaching Experience

Applicants must be:
- Preferably a Roman Catholic as prescribed by the Cagayan de Oro Network of Archdiocesan Schools (CONAS) of the Roman Catholic Archdiocese of Cagayan de Oro (RCACdO)
- Must be of Good Moral Character with pleasing personality.

HARD COPIES must be personally submitted by the applicants at the School Office.
