2 pcs. 1x1 Picture (Chemical Print)
3 pcs. Passport size picture (Chemical Print)
2 copies of Birth Certificate (PSA photocopy)
2 copies of Marriage Certificate (PSA Photocopy) for female applicant
2 copies of Form 137 for High School or SHS Graduate or Transcript of Records for Under/Graduate College
Medical Certificate stating you are Mentally and Physically Fit
Training Schedule: NO SPECIFIC DATE (To be announced if there's available scholarship allotment from TESDA)
