📢 LIBRARY ANNOUNCEMENT

SUBJECT: Application for Library Card

The Campus Library is now accepting applications for Library Cards for all students, faculty, and staff for the Academic Year 2025–2026. The Library Card will serve as an official identification for availing library services such as book borrowing, computer use, and access to digital resources.

📌 Requirements:

Fully accomplished Library Card Application Form
1 pc. 1x1 ID picture (white background) for Online Applicant
2 pcs. 1x1 ID picture (white background) for Walk-In Applicant
For Students: Proof of enrollment, Certificate of Registration (COR), First Semester 2025-2026
For Faculty & Staff: Employee ID

🗓 Application Period: Monday to Friday
🕘 Time: 8:00 AM – 5:00 PM
📍 Venue: Campus Library – Registration Desk
🗓 Deadline: OCTOBER 30, 2025

🔔 Important Reminders:

The Library Card is non-transferable.
Only those with a Library Card may borrow library materials.
Lost Library Cards are subject to a replacement fee.
