A. For New Enrollees and Transferees:
1. Transcript of Records
If unavailable, submit: (a) Certified True Copy of Grades and (b) Certification of Graduation
2. 2x2 ID picture (colored picture on a white background, with name tag)
3. Birth Certificate from the Philippine Statistics Authority
4. Certificate of Employment or Duty Status (for self-employed or unemployed applicants, a letter attesting current employment status)
5. For transferees only: Certificate of Honorable Dismissal or Transfer Credential, from the last school attended
6. Diploma [Optional, for now]
7. Certificate of Good Moral Character, from the last school attended. [Optional, for now]

B. For Returning Students:
1. Updated 2x2 ID picture (colored picture on a white background, with name tag)
2. Updated Certificate of Employment or Duty Status (for self-employed or unemployed applicants, a letter attesting current employment status)
