If your policy has changed to any of these programs, you MUST have your new card with you at your next visit. There is different billing information that we need to process your claim. If you do not have your card, you will have to be rescheduled. Also, your P*P may have changed and many of you will need a new referral. Please be aware of these changes and make sure we get the proper referral for your visit. This will help expedite this change.
