During the inspection we heard a new "thumd" noise . We convinced the client to take the hoist down and this is what we found after taking the hoist gearbox out.
The drum bearing inner race broke the end off.
This is a Demag.
now while we have it down we go the extra mile and replacing all the bearings.
Lesson : Never over look the significance of a new and unfamiliar noise or sound during inspection
