🧠💻☎️Coaching for Caregivers focuses on your health and well-being. Caring for someone living with dementia can be meaningful and exhausting. Individual health coaching is designed to help you maintain and improve your own physical and emotional health. This program does not teach caregiving skills.☎️💻🧠

How can I qualify?

• Be a caregiver for someone with dementia
• Complete four required forms for the program sponsor. All information is confidential.

If you are age 60 or over and live in our 14-county service area, we can help! Case management will provide a ONE-TIME payment on ONE utility bill of up to $800 - Water, Gas, Electric or Propane.

The bill must be in the client’s name or spouse’s name.
