It seems that increasingly, doctors are expected to take on what some call the “BSM” – business school mindset.

The issue is, doctors attended medical school, not business school.

And yet, the way the industry is going (and where it continues to head unless there’s a significant change), the business school mindset is what is being promoted and pushed – much to the detriment of the doctor, and ultimately, the patient.

We believe that the path forward to improving the patient-doctor experience is an anathema to a system that prioritizes money over medicine, profits over patients. Care cannot be effectively delivered when cost-cutting and controlling are top priority, when the doctor is as often an administrator as they are a physician.

We need a shift, and fast, one that focuses on the doctor’s ability and commitment to their patients, one that appreciates and supports their education and experience, and one that makes all elements of seeking care – from diagnosis to treatment to recovery – seamless, uncomplicated, and transparent.

Victoria Silas, MD, recently wrote an article titled “Groomers at PetSmart have more autonomy than physicians.”

She lamented over the fact that so many doctors she knows don’t have the option of scheduling longer appointments for patients who need it (because their time slots are mandated by some other entity, or because pressure from higher up requires they see more patients in less time). She wrote over the loss of freedoms doctors are suffering, and how that is directly contributing to physician burnout.

The systemic issues, she says, must be addressed, must be fixed. It’s not enough to tell doctors that they simply “need to manage their stress better.”

The end of her article read, “Systemic problems need system solutions.”

How important is trust in a doctor-patient relationship? In what situations can mistrust happen?

It can rear its head in many ways: in incorrect paperwork and documentation, in misunderstandings of treatment plans, in financial variables. When third parties and administration burdens get between a patient and doctor, that’s when mistrust often happens.

The implications of a poor doctor-patient relationship can be severe. With mistrust, patients might hesitate to seek medical care; they may be suspicious of their doctor’s recommendations; they may become pessimistic or altogether hopeless over a positive outcome.

It’s so important to restore the importance and value of the doctor-patient relationship, because it’s in that relationship that concerns are acknowledged, hope is gained, and recovery is made possible.

I recently came across a health insurance site where the question “what should you do if a claim is denied?” came up as one of its
FAQs.


The response?

If a claim was denied because a service or a provider was not covered by the policy, “there is not a lot you can do to fight the denial,” they wrote. “It would more than likely be a waste of time.

“To prevent anything like that from happening again, you should always closely look over your policy.”

What a mortifying answer.

So often, doctors spend an inordinate amount of time advocating for their patients, appealing the denial, explaining in as much detail as possible their arguments against the denial. Phone calls are made. Supportive documents are sent. And time is lost.

For better or for worse, patients today have so much access to medical information, simply using search engines and word of mouth.

Symptoms are googled every day. Possible diagnoses – from the simple to the silly to the tragic – come up after a few clicks. Treatments, too.

Patients of course bring these to appointments and into conversations with doctors, who often recognize the well-intentioned reasons behind patients trying to self-diagnose, but who must balance expectations of care with what their education and experience have taught them is safe, reasonable and sound.

For this reason (and many others), the doctor-patient relationship is so important.

It cannot be transactional. It cannot be a process during which the patient tells the doctor what he or she must do. In the same vein, no insurance company – who is not even present in the room during examination or diagnosis – should be telling the doctor what can or can’t be done either. When others interrupt the doctor’s process, when the doctor’s education, experience, expertise, and skill is questioned, it can have disastrous consequences.
