Punctal plugs can be a great treatment option for dry eye patients and I do use them in my practice. Your puncta are the little drainage holes in the corner of your eyelids. Plugging them keeps your tears on the surface longer instead of draining away. For the right patient they can be really helpful.

But they don't work for everyone. And sometimes they actually make things worse.

What you're seeing in this video is the irritation that a silicone punctal plug can sometimes create.

I've had patients come to me who have been dealing with that scratchy, irritated feeling for months and couldn't figure out why their dry eye felt worse after treatment. Sometimes this is exactly what we find.

It doesn't mean plugs are bad. It means dry eye treatment isn't one size fits all. And part of my job is looking at the whole picture, not just placing a plug and sending someone on their way.

If you’ve ever sat in my chair and thought “wow, why can’t my glasses look like this”… you are not alone. I hear this all the time, and there are actually a few real reasons why it happens.

The quality of your lenses matters more than people realize. Not all lenses are made the same way. Higher quality lenses are ground with more precision, have fewer distortions, and give you a cleaner, sharper image. The difference between a budget lens and a premium lens is something you can actually see, literally.

Anti-reflective coating is not optional, it’s essential. Without it, light bounces around inside your lens and creates glare, haze, and that slightly foggy feeling, especially at night or in bright environments. A good AR coating lets more light through cleanly, which is what makes vision feel crisp and clear.

The measurements in your glasses have to be exact. The optical center of your lens needs to line up perfectly with your pupil. If your frames were adjusted incorrectly, stretched over time, or if the measurements were even slightly off when made, your vision won’t feel as sharp as it did in the exam room. Small errors in those numbers make a bigger difference than most people expect.

If something feels off, it’s worth looking into.
