At enrollment, this young mother was carrying the weight of raising three children on her own while navigating financial instability, unreliable transportation, inconsistent employment, and the ongoing stress of keeping her household afloat. As a single mother of children ages 2, 8, and 9, she faced constant pressure trying to balance parenting, work, and basic survival with very limited support.

Although the family had housing, they were living on extremely low income and had experienced multiple evictions in the previous year, leaving them at continual risk of losing stability again. The participant also struggled with anxiety and depression while trying to provide for her children and maintain employment.

After connecting with Project Unity’s Brazos County Family Resource Center and later enrolling in the HOPES Parents as Teachers program, she began receiving consistent support through home visits, care coordination, resource navigation, and basic needs assistance. Early interventions focused on helping stabilize the household through food assistance, clothing support, and connections to community resources while building a foundation for long-term stability.

Over time, she became increasingly engaged in services and remained determined to improve her situation. She pursued employment opportunities and eventually secured more stable full-time work while continuing to navigate the challenges of parenting and limited child care options.

In the fall of 2025, the family faced another major housing transition and temporarily lived in a hotel while waiting for permanent housing. During this difficult season, her Community Health Worker (CHW) provided ongoing encouragement, practical support, and care coordination to help prevent another housing crisis. On October 15, 2025, she successfully moved into her own apartment, bringing a renewed sense of relief, security, and stability for both her and her children.

The CHW assisted with household setup needs and provided coaching through Pathways learning modules focused on avoiding eviction, financial management, budgeting, payment planning, and strengthening landlord relationships. These tools helped build confidence and practical skills that would support long-term housing stability.

When utility disconnection later threatened the family’s progress, the CHW coordinated multiple referrals and explored every available resource option. Through those efforts, the family received $441.80 in utility assistance, preventing service disruption and protecting the stability they had worked so hard to achieve.

Today, the participant has maintained stable housing, utilities, and essential household needs while developing stronger financial management skills and greater confidence in caring for her family. Her journey reflects the impact that consistent support, persistence, and community partnership can have in helping families move from chronic instability toward lasting hope and security.

When she first enrolled in services, this young mother was facing one of the most uncertain seasons of her life. At 37 years old, pregnant with her second child, and raising her family on her own, she was living in a maternity home with no income and very limited stability. Everyday concerns such as housing, food, transportation, and access to healthcare weighed heavily on her as she prepared to welcome a new baby into the world.

At the time of enrollment, she was unemployed and struggling with food insecurity while relying on SNAP and Medicaid to meet basic needs. The stress of homelessness, pregnancy, and financial instability created significant barriers to building a secure future for herself and her children.

Through the support of her Community Health Worker (CHW), she began taking meaningful steps toward stability and independence. Regular home visits and ongoing care coordination focused on helping her secure safe housing, improve financial stability, and maintain healthy prenatal and postpartum care.

During her pregnancy, she achieved a major milestone by securing full-time employment and successfully maintaining her job through the later stages of pregnancy. In April 2024, she delivered a healthy baby, marking the beginning of a new chapter for her family.

One of the most transformative moments in her journey came when she was accepted into the Flourish housing program. This opportunity allowed her to transition out of the maternity home and into a fully furnished apartment with rent assistance for up to two years. For the first time in a long time, she and her children had a safe and stable place to call home.

Throughout this transition, the CHW helped connect her to critical resources, including transportation assistance through OnRamp, food support, diapers, formula, and baby supplies. These services eased immediate burdens and allowed her to focus on caring for herself and her newborn.

Following delivery, she remained actively engaged in services and continued attending medical appointments and postpartum follow-ups. When she began experiencing symptoms of postpartum depression, she sought help and received medication support and education to better manage her mental health. Her CHW continued encouraging healthy routines through education focused on nutrition, preventive care, family planning, and positive parenting skills.

Although she later experienced a temporary period of unemployment, she remained resilient. With support, she successfully activated SNAP and TANF benefits, continued searching for employment, and maintained both stable housing and reliable transportation during the transition.

By the time she completed the program, her life looked dramatically different than it had at enrollment. She had stable housing, dependable transportation, active public benefits, established medical care, and increased confidence in her ability to provide for her family. Most importantly, she left the program with a stronger sense of hope and stability for herself and her children.

Her journey is a powerful reminder that when consistent support, community resources, and personal determination come together, lasting transformation is possible.
