Vaccines

MMR- Measles, Mumps, Rubella
TDap- Tetanus, Diphtheria, Petussis
Varicella- Chicken Pox
Tuberculosis

TB Test (in the last year)
Uniform

White Polo Shirt
Black Slacks
Black Non-Slip Shoes
Vaccines are shots you get when you are a child that can be retrieved from the health department. If it can’t be retrieved it will have to be completed.
