ICYMI: We’re highlighting an innovative approach to patient care that is taking shape in North Carolina.

The Duke Endowment and the North Carolina Healthcare Association recently launched Bridge to Health, a pilot designed to better support patients with complex care needs that arise from multiple chronic medical conditions, substance use disorder, mental health challenges, homelessness and/or poverty.

Six health systems across the state are working together to establish or expand hospital-based clinics that address acute medical, mental health and social needs for their highest-risk patients. Strengthened partnerships with primary care providers and community-based organizations ensure these patients have a continuum of care once they leave the hospital.

Creating lasting improvements in community systems requires that people and organizations learn, collaborate and work together toward shared goals. Cohorts can provide the environment to make that happen.

Through our School-Based Oral Health initiative and Healthy People, Healthy Carolinas program, The Duke Endowment learned valuable lessons about what helps cohorts succeed. Insights about leadership, readiness, peer connection, evaluation and more are helping us improve collaboration across the Carolinas.

Each year, 30-50% of licensed kin caregivers and foster parents quit, directly impacting children’s outcomes and sense of belonging.

This loss can be detrimental to the more than 14,000 children in foster care across the Carolinas in need of support, stability and connection with peers and nurturing adults.

Recent Davidson College graduate and Duke Scholar Thomas Lane reflects on how scholarship support gave him the freedom to follow his interests – from studying abroad in Zambia to preparing for medical school – and build lasting relationships along the way.

Underused church spaces can serve communities in new and meaningful ways.

A United Methodist Church property in Charlotte was transformed into affordable senior housing, connecting older adults with community, transportation and support services. The project, led by Wesley Community Development, shows how church properties are being reimagined to meet urgent community needs.
