The first exception to a naming convention gets made because there's a real reason for it. The second gets made because someone remembers that exceptions are allowed. By the fifth or sixth, the standard is just a document nobody follows anymore.

Drifting standards isn't a people problem. It's what happens when there's no system to catch exceptions before they compound.
