As a result, health plans struggle to engage with beyond 37% of their members independently. However, through partnership with LocalConnections for Health, health plan engagement is significantly enhanced to reach up to 80% of members.

How do we achieve this?

Our approach involves leveraging trained and certified community health workers who act as trusted guides to healthcare resources and social support. Our health workers connect to families in their communities through community-based organizations, telephone services, and face-to-face interactions.

Our focus extends beyond setting individual health goals; we meticulously devise workflows and implement tailored next-best actions based on each family member’s specific health needs. This approach ensures that health plan members and families receive personalized guidance, placing them on a path towards a brighter and healthier future.
