10 months post partum- pelvic floor physical therapy appointment #2 tomorrow. There’s nothing ‘wrong’ with me. Most people wouldn’t even say they had symptoms & certainly wouldn’t want to ‘waste’ their time going to an apointment(s) over what I’m experiencing.
Minimal p***c symphysis pain & occasional, minimal incontinence when sneezing.

I’m obsessed with teaching about the pelvic floor muscles! Huge fan of teaching patients and professionals. Coolest muscles ever 😉 Love this model too for educating!
In fact, the reason I became a pelvic PT 19 years ago was because of the obturator internus. 💥 I was a student in a pelvic health and ortho clinic. I was shocked at how many patients said “I have had back surgeries, injections, and years of manipulations...why is it when you touch THAT muscle inside of me, it makes me have the same pain? Why was this missed for so many years?” 💡💡 A huge light bulb went off. The pelvic floor muscles may be a reason for back or hip pain that’s being ignored!! Happens so often.
But, I have a confession to make:
Sometimes I feel there’s WAY too much focus on them. There. I said it. It drives me crazy.
Let’s get away from compartmentalized care. 1. We have to remember to address the whole body as a system working together and 2. Differentials are so important - and may be beyond your scope. Things are missed.
I saw 2 new pelvic pain patients already this week who had specific issues missed completely for years...because there was such a heavy focus on pelvic floor muscles (and abs and adductors) or doing additional surgeries. One patient said “why has no one ever touched there. That’s exactly where my pain has been since day 1 - and it wasn’t the obturator, after all... it was the vaginal cuff.
So, we have to remember to address all potential issues based on history, symptoms, what makes it better or worse, and specific palpation for reproduction of symptoms (if that’s what makes sense). Is it a condition or multiple conditions? ... more in comments.
