Phoenix Equipment Restoration

Cause & Origin as to why my facilities DVR Camera System dropped it's IP address and just power cycles on/off. Found (3) swelling capacitors on board (easy fix); however, thermal damage found to the USB port's 5V power input/output circuit which is tied to the unit's main 5v rail. Unit deemed KAPUT!

We performed another Cause & Origin (C&O) Investigation of a dryer fire today. This potentially deadly situation was caused by lint buildup within the flexible exhaust duct. The inner flexible coil of the exhaust tube catches lint with it's "accordion rings" causing lint to clog, which ultimately leads to this. Phoenix ER highly recommends that everyone replace their flexible exhaust duct with solid round ducting to avoid this potentially deadly fire hazard. Thankfully, there were no injuries on this one, except for the dryer and the wall behind it.
