This wasn't a freak accident. It was a predictable outcome of a high-risk situation that needed to be stopped before it started. Ask yourself:

🔴 Was there a safe way to do this task?
🔴 Did anyone stop and identify the hazards before climbing in?
🔴 Was there pressure — spoken or unspoken — to just get it done?

Safety isn't about following rules for the sake of it. It's about making better decisions BEFORE you're in a position where things can go wrong fast.

No load of material is worth a life. No job is so urgent that it can't be done safely.
