Hello families! We are getting into sports and camp form season and we just wanted to send out a few reminders. ⚾️🏈🏀⚽️🎾🏐
- For the ISHAA form to be filled out, the patient must have a physical on file after April 1 of the year participating in the sport.
-For the ISHAA form to be filled out the first page must be fully completed by the parent and turned in with the rest of the packet
-All forms must have the patients name and DOB completed by the parent before we are able to complete the rest
