Take a close look at the wire. Notice there are little copper beads coming through the insulation. Once again improper circuit protection allowed wire inside a pipe to overheat to point where the wire began to melt like little beads.

Several issues here. 1-This circuit was not properly protected with a 20 amp breaker instead of a 15 amp that the wire was rated for use. 2-Even though this much damage occured it was very well controlled because the proper metal box was used and all connectors and openings were secure and in place.
This could have had a very different outcome if just one part of the system did not perform properly.
