The Care Coordination Manager coordinates, providing outreach, education, and access to resources.

This entrepreneurial, self-starter, and collaborator will support the Care Coordination Manager and the team in building, implementing, certifying, and sustaining the HOPE Pathways Community HUB in Fresno and Kings County.
