3 Key Exercises to Improve Shoulder Function and Reduce Pain from Shoulder Impingement

Encourages proper shoulder blade motion and upward mobility.

✅ Tip: Slide arms up a wall with your thumbs pointed backward, keep tension light.

2. Isometric External Rotation (with Towel Roll)

Reduces inflammation while activating stabilizers.

✅ Tip: Hold a light resistance band or towel against a wall, push out gently and hold.

3. Prone Scapular Retraction (on pillow or bench)

Trains shoulder blade stability for long-term pain relief.

✅ Tip: Lying face down, gently squeeze shoulder blades together and hold 3 seconds.

When recovering from an injury, surgery, or managing a chronic condition, many clients ask:
“What’s the difference between physical therapy and medical exercise training?”
It’s an important question—and one that highlights a growing need in today’s healthcare system. While both physical therapists (PTs) and Medical Exercise Professionals (MedExPROs) help clients improve function and quality of life, they do so at different points along the care continuum and with clearly defined roles. Here’s how to understand the distinction:


✅ 1. Physical Therapy: Clinical Rehabilitation
Physical Therapy is a licensed medical service. PTs evaluate and treat impairments, pain, and functional limitations caused by injury, illness, or surgery. They use manual techniques, modalities (like electrical stimulation or ultrasound), and therapeutic exercises to help patients:
● Regain range of motion

● Reduce inflammation and pain

● Heal injured tissues

● Restore daily function


PTs work under physician referrals or direct access laws and focus on clinical outcomes during the acute or subacute phase of care.

Medical Exercise Training (MET) is not treatment or therapy. It begins when a client is medically stable and discharged from therapy or cleared by their physician, but still needs structured exercise to continue improving.
A Certified Medical Exercise Professional:
● Designs personalized, safe, and progressive exercise programs

● Focuses on long-term function, strength, endurance, and mobility

● Addresses chronic conditions like arthritis, diabetes, hypertension, and joint replacements

● Works closely with healthcare providers to support continuity of care


Medical Exercise Professionals do not diagnose or treat, but instead focus on fitness-based programming for medically stable clients.
