HELP US IMPROVE THE HEALTH OF OUR COMMUNITY

Damar Health is conducting a Community Health Needs Assessment to better understand the health concerns, challenges and priorities affecting residents living in or near Decatur Township.

This confidential, anonymous survey takes approximately 15 minutes to complete and will help identify community perspectives related to quality of life, access to services, and physical and mental health needs. The information gathered will play an important role in helping improve health resources, patient care and community services for the area.
