In 2017, five employees overdosed and died in the workplace in Kentucky, representing 7.4% of the state’s fatalities last year. As drug use in Kentucky rises, employers are becoming more likely to have members of their work force misuse drugs and overdose while performing work duties.

OSHA has determined that there are four main safety hazards, excluding transportation incidents, that account for a majority of all construction worker deaths each year on the job-site. Nicknamed the "Fatal Four" by OSHA, these include falls, electrocutions, being struck by objects, and getting caught in or between hazards. Statistics show in 2016, 63.7% of all fatalities at construction sites were from one of OSHA's Fatal Four.

What does FACE stand for? It stands for Fatality assessment Control evaluation.
Our team investigates work place fatalities across the state of Kentucky. Reports are then created for prevention. The reports are then distributed to a list serve made of similar industries so they can be used as tool box talks, teachable moments and to bring awareness of the hazards. Reports are published to KIPRC website as well as the CDC/NIOSH website for free usage.
