Hybrid Position.
This role involves gathering information, accurately documenting in the EMR, and reports to RN. You will develop personalized care plans for every patient under RN oversight
Goals
• Initiates phone contact with eligible Medicare patients in attempt to enroll them in CCM/RPM programs on behalf of physician practices
• Meets or exceeds outbound connect rates and enrollment objectives
• Completely and accurately educates eligible patients on the benefits of CCM/RPM programs
• Obtains consent from patients electing to opt-in to the programs
• Data Entry and documentation of patient interactions in system: HumHealth
• Maintains a thorough and up to date understanding of program details, benefits and logistics
• Adheres to applicable regulatory and company guidelines
• Works consistently and persistently to build patient enrollment
• Monitors vital signs and reports out of range vitals to the RN
• Assist with occasional delivery of RPM devices
Requirements
• CNA, MA, MedTech
• Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.
• HIPAA-Safe space to do your work. Others are not to be able to hear conversations or view screen.
• Your own computer with a reliable, fast internet connection, able to have multiple windows open at once. Headphones with microphone recommended.
