Dear Fit for Life Patients,

We’ve had several patients message or text us with confusion. I want to be clear that Dr. Lindsay Dodd is still at the office location at 2960 Immokalee Rd. She WILL remain there as will Adi, our massage therapist. You can call 239-513-9800 to make an appointment or use our Jane App for booking online.

All of the above providers are committed to the same excellent healthcare you have always received at our office. We also still have our adorable store with local small business vendors, and our staff remains intact with the exception of Jill who is going to retire to spend time with a new grandbaby on the way. We are very excited for her. She will pop in now and then as we need her and we are ever grateful for her support and dedication to our patients. 

As for the other providers, by Florida law, it is their responsibility to have reached out to you as their patient either via social media, newspaper (I know. Who reads a newspaper anymore but that is the FL statute requirement.), email or some other means of communication to alert you of their new practice location. They are the “records owner” although a copy of your medical records will remain at Fit for Life per FL law.

“Statutory Requirements Of What To Do When A Physician Changes Offices:

Under Section 456.057, Florida Statutes, a “records owner” means any health care practitioner who generates a medical record after making a physical or mental examination of, or administering treatment or dispensing legend drugs to, any person; any health care practitioner to whom records are transferred by a previous records owner; or any health care practitioner's employer, including, but not limited to, group practices and staff-model health maintenance organizations, provided the employment contract or agreement between the employer and the health care practitioner designates the employer as the records owner. Section 456.057(2), Florida Statutes. The “records owner” may or may not be a physician, it can be the employer of the physician. The statute requires the “records owner” to notify patients of the retirement, closing or relocation of a physician.
