The Hope Center Development Corporation(HCDC) is pleased to partner with The Connect2Care program offered by United Way of New York City’s Preventative Access Program. The goal of the Connect2Care Program is to connect chronically ill, at-risk low-income New Yorkers to health and social services, ensure they are linked to a medical home and improve access to health resources, screenings, out-of-hospital preventive care and social support services. The program focuses on addressing key health problems including key diseases such as diabetes, cardiovascular disease, maternal morbidity, obesity and hypertension). The program will provide direct preventative health screening services to low-income and under-insured individuals.
