This session is designed to share the Multnomah County CHIP and collect feedback from the disability community. All attendees much live or work in Multnomah County. Please feel free to invite other members of the Multnomah County disability community.
A Community Health Improvement Plan, or CHIP, outlines the community-identified health priorities, goals, and strategies needed to build a healthier community. Multnomah County’s 2026-2026 CHIP will help to align efforts across public health, behavioral health, and community partners to support what matters most to Multnomah County residents.
Strategies will focus on prevention and health promotion, and work toward decreasing preventable deaths, improving health outcomes, and ending unfair gaps in health. The CHIP will be implemented directly by MCHD and in collaboration with community and regional partners. Progress will be monitored ongoing by a Community Advisory Group.
