PARTICIPANT’S FULL NAME (AS SHOWN ON THEIR PASSPORT OR THE NAME THEY ARE APPLYING WITH FOR THEIR PASSPORT)

NAME, EMAIL, ADDRESS, DATE OF BIRTH, AND PHONE NUMBER

THIS STEP IS REQUIRED EVEN IF THE PARTICIPANT HAS BEEN ON A TRIP WITH US BEFORE.

SERVE Botetourt it THIS Sunday after Second Service! This is an amazing opportunity for students to serve our local community through the Veritas Collective, also known as the Old Colonial Elementary School. We plan to meet at the school at 1:00 PM and go until 4:30 PM.
