Was finally able to put the last few finishing touches back together on this one.

This customer’s 1998 12-valve Dodge Cummins diesel had a South Bend dual disk installed at a different shop a few months ago. The vehicle experienced an issue with the 5-speed NV4500 transmission that is common to many NV4500 models, where the main shaft 5th gear nut in transmission backed off despite having an upgraded lock nut kit, leading to the loss of 5th gear and damage to the gears.
During the disassembly, an inspection revealed that two of the four bolts holding the transmission to the bell housing were missing, one bolt was loose enough to be removed without tools, and only one bolt was still tight, as evidenced in the pictures posted in the comments.
Accidents and mistakes can happen to anybody, But be sure when having your vehicle repaired, the shop or Individual completing the repair is knowledgeable and thorough with their work.
